Isotretinoin is a retinoid indicated for the treatment of acne and has been associated with several effects, including muscularskeletal symptoms such as arthritis, arthralgia and myalgia. The development of sacroiliitis is a rare side effect and a small number of cases have been described in the literature. The pacient M.G.P., 17 years old, male, was diagnosed with sacroiliitis after having started using isotretinoin, he presented serology negative for HLA-B27 and did not have any other infectious predispositions for the development of sacroiliitis. From the second week of interruption of isotretinoin use the symptomatology was significantly reduced.
