Background: A rare case of thrombocytopenia. Case History: A 66-year-old man comes to our observation, sent to us by the emergency room, for rectal bleeding and severe thrombocytopenia. The patient had undergone recent orthopedic surgery for knee replacement infection and enoxaparin therapy. Discussion: The patient underwent replacement therapy with platelet pool and corticosteroid therapy with only partial recovery of platelet values. Therapy with enoxaparin has now been discontinued. Colonoscopy revealed a colon growth. It was possible to obtain a recovery of the platelet values only after the sulfamethoxazole and trimethoprim was suspended.
