Statement of the Problem: Acute spontaneous spinal cord hemorrhage is rare disease and may present with acute onset of pain. The most cases have specific etiologies such as trauma, iatrogenic causes, vascular malformations and bleeding diatheses. This article aim to report one case of acute spontaneous spinal cord hemorrhage and review the relevant literature to present etiology, clinical manifestation, diagnosis and treatment. Recent advances in pathophysiologic mechanisms are also discussed.
Methodology & Theoretical Orientation: We performed the patient symptom, spinal cord magnetic resonance imaging (MRI), therapeutic and prognostic features. Findings: A 42 years old female visited the department of neurology emergency for sudden-onset back pain, paraplegia and incontinence. Spinal cord MRI revealed intramedullary hemorrhage at the C7-T4 level. And MRI diagnosis was spinal cavernous malformation. Since the patient rejected surgery, she performed rehabilitation every day for 30min. Her symptoms were gradually recovered and was transferred to a rehabilitation facility on hospital for three months. On discharge she was able to walk and void independently follow-up for 6 months.
Conclusion: Our case suggests that it is important for early rehabilitation in acute spontaneous spinal cord hemorrhage without operation. Physical therapy is an effective treatment for improving disabilities with intramedullary spinal cord hemorrhage. Future studies should aim to research the standardized management guidelines for spinal hemorrhage.
