The control of the nasal tip contour is the key component of successful rhinoplasty. Many commonly used nasal tip techniques can pinch the tip structures if an overemphasis is placed on narrowing. These techniques isolate the dome region of the nasal tip and can create an undesirable shadow between the tip lobule and alar lobule. The primary objective of this study was to describe how to avoid surgical appearance of the nasal tip and how to end up with a natural looking nose. Study designed as a retrospective chart review. We include 50 patients in this study age 20 to 50 years old of both sexes. For refining the nasal tip we used multiple techniques, some patients’ tips refined using suturing technique only. The second group refined using suturing and cartilage division without adding any grafts. Third group were refined with suturing, dividing the cartilage and grafting. Tip sutures are powerful tool that can deform the cartilage ending up with a pinched tip with the characteristic demarcation between the tip and the alar lobule. For contouring the nasal tip, we should think structural, focus more on shadows and highlights and less on narrowing the nasal tip to avoid ending up with a pinched tip. Contouring the tip in this manner will look more natural leaving stronger nose for the future that can withstand the forces of scar contracture which negatively affect the rhinoplasty long term outcome. Suturing technique and dividing the cartilage frequently used to flatten the lateral crura and eliminate tip bulbosity. When dome sutures are not enough we add grafts to create fine and smooth shadows and highlights from the tip lobule to the alar lobule. This form of restructuring can create a natural-appearing nasal tip with horizontal tip orientation continuing out to the alar lobule.

He did Post Specialty Training in Rhinology in Munich LM University and another Fellowship in teaching hospital of Insubria University in Varese Italy. He is dedicated for rhinolasty surgery.
