The nose is a functional and aesthetic structure located in the center of the face. The vast majority of studies focus on treatment related to the dorsum and nasal tip. This study aims to describe the treatment options of the cartilage graft in the alar region in rhinoplasty. A review of studies was carried out, in which studies related to the treatment of the alar region in rhinoplasty using cartilage graft were included. Studies related to other anatomical areas of the nose, congenital defects and flaps related to nasal area treatment were excluded from the analysis. A survey was carried out in PuBMed and SCIELO, from Jan’ 1998 to Nov’ 2018, in the English, Spanish and Portuguese languages, using the keywords cartilage and alar. 2174 studies were found, after evaluation according to criteria, were excluded the replicated studies that were not related to the subject. We found 28 studies related to the proposed question. The nose is a complex structure on the face, the alar region should be especially attentive due to the importance of its functional and aesthetic aspect, and the preservation and use of autologous cartilage is an excellent option for the treatment of this region
