Background Salmonella infection have a prevalence of 2% in general population but only of 0.2 %, in pregnant. Case Report A 27-yo pregnant woman come to ED at 38 weeks’ gestation due fever and diarrhea (15 watery stools a day). She was hypotensive, tachycardia and hypovolemic state. At blood analysis there was Na 130, K 3.0, PCR 10 mg/dl but normal WBC and PCT. Stools culture resulted positive to Salmonella Typhi. Thus, the patient was promptly treated with electrolyte fluid solutions and antibiotic (ampicillin/sulbactam 1gX3). After 4 days from admission, she gave birth the child without complications. At 8 days they were discharged in good conditions on antibiotic therapy. Conclusion Hormonal changes during pregnancy may impair the cell Mediated immune response increasing the susceptibility to infections like salmonella. This is an important issue and the history of diarrhea in a pregnant woman may suggest the search for Salmonella as soon as possible for a prompt diagnosis and having a good outcome avoiding complications and death of new-born.
