Many surgeons consider that the management of the nasal tip by a closed approach does not allow an adequate visualization of the area, for this reason they use external rhinoplasty more frequently, being its distinctive the transcolumular incision, which does not always leave an adequate scar. In Mexico, Dr. José Guerrerosantos in 1990, published the article ".Open Rhinoplasty without skin-Columella incision". The present conference shows the management of the nasal tip by this access route, which provides an excellent vision for the correct management of the structures but without the inconvenience of a scar in the columella. The step-by-step approach will be described, as well as some techniques to improve the projection, definition and rotation of the nasal tip, illustrating the segments with short videos (less than 60 seconds each) and ending the presentation with casuistry and pre-op photographs of operated patients through this approach.
