Hypertelorism is a rare congenital craniofacial anomaly associated always with some form of nasal anomalies sometime quite severe. The rhinoplasty in these patients differs from the standard purely cosmetic nose as staged procedure is necessary to reach an aesthetic nose and normal facial look. The author describe in details about the methods adopted, the difficulties encountered and the results obtained with a follow up of over 15 years. The first stage is reconstruction of the nostrils in case of presence of a defect. The second stage concerns the broad tip. It is done at the same time as the correction of the hypertelorism. The nasal bones are brought together and the far apart left and right domes and are approximated so as to form a pleasant projecting tip. The third stage is done at around age 15 years and is augmentation of the dorsum by a bone or rib graft and lateral osteotomies whenever necessary. Fine touches could be done a couple of years later if needed. The long term results and follow up for more than 15 years is presented.
