STUFFING THE MACULAR HOLE - various stuffing material for Macular hole Vitrectomy surgery

Abstract:

PURPOSE: To describe the surgical procedure for closure of idiopathic primary macular hole using various stuffing materials like ILM, Pedicle graft of ILM, wet amniotic membrane, autologous conjunctival graft, autologous platelet rich plasma, posterior lens capsule, autologous retinal graft in pars plana vitrectomy surgery. SETTING VENUE: Rajput Eye Hospital, Ahmedabad, Gujarat, India . All cases were done by single surgeon Dr. Alpesh Rajput. METHODS : All cases of Vitrectomy Surgery for macular hole closure were done with 25G system. In all cases primary vitrectomy was done after inducing PVD with TA. Then BBG dye injected to stain ILM. ILM peeled off in all cases with ILM peeling forcep. Macular hole stuffing was done by using different materials like ILM, Pedicle graft of ILM, wet amniotic membrane, autologous conjunctival graft, autologous platelet rich plasma, posterior lens capsule, autologous retinal graft in different cases. At the end of surgery 14-16% C3F8 gas was injected & all patients were given prone position. RESULTS: We achieved 100 % anatomical closure in all cases irrespective of stuffing material used. Although visual outcome is not much in wet amniotic membrane and autologous conjunctival graft. Visual outcome was good in ILM stuffing case and posterior capsule graft and autologous platelet rich plasma. CONCLUSIONS: Various stuffing materials like ILM, pedicle graft of ILM, wet amniotic membrane, autologous conjunctival graft, autologous platelet rich plasma, posterior lens capsule, autologous retinal graft gives excellent anatomical closure and reasonably good visual out come in idiopathic primary macular hole vitrectomy surgery.
