Thoracodorsal flap for reconstruction of Type II axillary contracture

Abstract:

Objectives: The aim is to reconstruct axillary contracture by a well-padded pliable flap with known vascular pedicle, good arch of rotation with less donor site morbidity and increase range of movement of involved joint.

Methods: Twenty two patients were operated for this prospective observational study conducted in the Department of Plastic Surgery, Dhaka Medical College and Hospital from Jul’ 2014 to Jun’ 2016. All patients with Type II axillary contracture was released and established the maximum abduction of shoulder joint and then closed with Thoracodorsal flap.

Results: Axillary contracture of the study involves predominantly (75%) in right side and 14 (70%) had 60-900 shoulder abduction movement beyond the contracture over the joint. The average vertical and horizontal dimensions of the defect after release were 11.50±3.154 cm and 7.50±1.792 cm respectively. The average vertical and horizontal dimensions of the flap were 10.48±3.389 cm and 7.25±1.410 cm respectively and on average 107.380 angle of rotation of flap is required to cover the defect. Final outcome shows 85% of the patients exhibited excellent outcome with 10% patient’s outcome was considered satisfactory.

Conclusion: Thoraodorsal flap is a useful flap to reconstruct the defect after release of type II axillary contracture and can provide satisfactory functional outcome with negligible post-operative problem.

Biography :

Afrina Sharmin is working as an Assistant Professor in Z H Sikder Womens Medical College Hospital in Plastic Surgery Unit of Department of Surgery, Dhaka, Bangladesh. She was working as an Assistant Surgeon and Coordinator of Comprehensive cleft and craniofacial unit in Sheikh Hasina National Institute of Burn and Plastic surgery from Jan’ 2016 to Nov’ 2017. Before that she has experience to work in a NGO Acid Survivors Foundation who dedicatedly works for the acid victims for four years. She has completed her Fellowship in Plastic and Reconstructive surgery in 2016 from Bangladesh College of Physicians and Surgeons and Masters in Public health from State University of Bangladesh. She has attended an advanced training in cleft surgery at Great Ormond street hospital in 2016. She regularly visits the Asian countries to attend the conferences to develop and practice international standard protocol feasible for developing countries, to work with local surgeons and to assist in their development of multi-disciplinary teams. She is a Life Member of the Society of Plastic surgeons of Bangladesh, Society of surgeons of Bangladesh, Asian society of Mastology. She has two publications in Bangladesh journal of plastic surgery and Journal of surgical sciences.
